The Skeleton Head is a mask. It was available as the November 2019 Dark Mode item, via the /darkmode code.
It matches with the Skeleton Body.
At it's original time of release, it was a head item, but was later changed to be a Mask, allowing it to be worn with other head items.
A similar item is the Skeleton Mask 
, the only difference is that the former doesn't have skeleton ears.
